  • This is a PRELIMINARY APPLICATION ONLY. Because the demand for Service Dogs is great, the application process involves MANY steps and a LOT of paperwork to be completed. We utilize this tool to determine the first level of "fit" for you and one of our dogs. Please watch your email for IMPORTANT communications from ous regarding the next steps. Each step is time-oriented, and you cannot be placed on a waiting list until ALL STEPS are Completed.

Completing this application does NOT obligate you or Leprechaun Lane to the Placement of any dog, puppy, or prospect. Completion of this application is NOT a guarantee of acceptance into our program. Completion of this application is NOT a validation of need or suitability for a Medical, Therapeutic, Working, or Fabilities dog or prospect.
